Preparing For Your Visit
1Please arrive 10 minutes prior to your scheduled appointment time in order to complete any last minute registration details.
2Please bring your insurance cards, a picture ID and a list of medications you are taking.
3All copayments are required at time of service. Patients who do not have their copay will be rescheduled.
4If you need to reschedule your appointment, we ask that you give us 24 hours’ notice. If you fail to notify our office, you may be charged a no show fee of $50.00.

How often will I receive a statement from Churchland Internal Medicine?
At least once a month, you will receive a statement that lists what your insurance company still owes and what you owe. Statements continue until all payments are made in full.
Why does the statement show a total account balance when I have insurance coverage?
If your insurance plan does not cover the services you received, you are financially responsible for your charges.
What happens if I cannot make the payment in full?
In most cases, we can help establish a payment plan depending on your balances due. Partial payments made toward your balance will not stop collection activity unless you have made payment arrangements with us. Please call Patient Billing Services to discuss payment options: (757) 484-5113

What happens if I see a mistake on my bill?

How can I get billing information for my adult child?
Your adult child has to sign a “release” for us to be able to provide you with any information about their account.
Why are my bills so high — I already paid my co-pay at the time of the visit?
Deductible and co-insurance requirements per your contract benefits may be the additional responsibility.
Why am I getting calls from a collection agency?
